Please find the New Patient Paperwork that you will need to have completed before your child’s first appointment. It is very important to fill out all forms completely so we can better serve you and your family. You can send the completed forms to our office by email to office@sonoransuntherapy.com or fax the forms to our office at 623 321-1981. It is very helpful to have these forms (48 hours) prior to your appointment so our therapist can prepare for your child’s appointment. You can also complete these forms in office, but you would need to arrive 30 minutes early to your appointment time.
